Patient Centered Medical Home


As a Patient-Centered Medical Home practice, we are dedicated to building a trusting, collaborative partnership between your child, your family and our primary care team of health professionals in order to oversee your child’s health and well-being. Our goal is not only to work together but also help empower you and your family to take an active role in the health of your family by providing support and the self-management resources that you need to succeed.

As part of that effort, we have a patient portal that allows you private access to your chart information, medication information, educational resources, and lab/test results. For more information, please see us at the front desk.

As Your Primary Care Team We Will:

• Take the time to learn about your child and your family, including your life situation and health goals.

• Treat your illness – whether it is short term or chronic – and take care of your overall well-being.

• Ensure that you are current with all of your screenings, tests, and vaccinations. Should your health needs change, we will work with other providers in your care team, such as a specialist, to coordinate your care.

• Be available after-hours by phone in case of emergencies.

• Notify you of test results in a timely manner

• Communicate in a way to help you understand your child’s condition and possible treatment options.

• Listen to your concerns and answer any questions that you might have regarding medical care.

• Help you make the best decisions for your child’s care.

• Provide resources and information to help you learn more about your child’s condition and help your family lead a healthy lifestyle. This includes support groups, classes, printed materials, and other services.


As Our Patient, We Ask That You:

• Work with us as a partner in your child’s care.

• Notify us of any changes in medications, supplements or remedies at each visit.

• Keep scheduled appointments. Please call as soon as possible if you have to reschedule.

• Learn about your child’s health condition and what steps you can take to help your child stay as healthy as possible

• Help us develop a treatment plan that will work best for your child. If there are issues that will make it hard to follow the plan, please discuss those with us.

• Have your child take medication as prescribed.

• Call us after hours if your child has an issue that is emergent and cannot wait until the day.

• Contact us before going to the emergency room so someone who knows your medical history can care for you.

• Learn about your health insurance coverage and benefits. If you have questions, please contact our billing office.

• Provide feedback when possible about our care so that we can improve on our services.

Monday – Thursday: 8 a.m. – 5 p.m
Friday: 9 a.m. – 5 p.m.
